Sharp left neck and shoulder pain radiating down the left arm
Severely limited and painful left cervical rotation; reproduces radicular symptoms
Cervical extension reproduces symptoms
Numbness and tingling in left thumb, index, middle finger, and intermittently the palm
Aching in the left triceps
Wakes from sleep at night, especially with head extension while lying down
Mild weakness in the left arm and hand
Left arm and chest muscles tremble during flexion or exertion on the left side
Immediate cramping in shoulder and arm when stretching or closing the shoulder
Upper back and shoulder blade muscles cramp up with movement

Patient-Reported Efficacy
Tizanidine and Cyclobenzaprine cause drowsiness. Prednisone provided significant relief but was short-lived (~1 week). Methylprednisolone Dosepak did NOT work. ER-administered 60 mg Prednisone was the most effective intervention. No other medication has provided adequate pain control.
